All temporary food service applications for the St. Patrick’s Day celebration in Savannah must be faxed to the Chatham County Environmental Health office at 912-356-2969 or delivered in person to the Chatham County Environmental Health office at 1395 Eisenhower Drive on or before 5 p.m. on Tuesday, March 3.
In addition to completed application forms, additional documents are required including:
A $60 permit fee is required.
Effective immediately, Verification of Residency for Public Benefits affidavits must be collected from operators/owners of all new facilities that require permits from the state of Georgia or a specific county. This includes Food Service, Tourist Accommodations, Body Art, and Public Swimming Pools that operate for a fee. These forms must be submitted with the respective permit application and notarized. Verification of Lawful U.S. Residency for License Application
Applications must be submitted for new food service establishments, mobile food service units, and temporary food service establishments. Completed applications, along with fees, should be returned to the Environmental Health office of your county health department.
